As we watch our loved ones age, it can be difficult to make self-reliance a priority for those who begin to require some assistance. However, it’s important to realize that removing an elderly loved one’s ability to do things for themselves is not only mentally harmful, but it can have an effect on their physical health too.

Maximizing Independence at Home

PACE, or Program of All-Inclusive Care for the Elderly, is a comprehensive model of care created to provide healthcare and recreational resources for the elderly. Catholic Health recognizes the need for seniors to remain active and independent, which is why we created our own PACE program. Our LIFE (Living Independently for Elders) program offers supervised adult care for seniors who may need regular assistance, while allowing them to remain in their homes.

The LIFE program not only provides a solution for families who may be deciding on where to place their elderly loved one, but it reduces unnecessary nursing home admissions, lowers hospitalization and re-admittance rates, improves participant’s quality of life, and can provide relief from the stress of caregiving. LIFE also provides participating seniors with high-quality specialized care, transportation to our LIFE centers and any medical appointments, socialization, and home care services all in one place.

A Different Kind of Care

More than 1 in 5 Americans are unpaid family caregivers to a loved one. Catholic Health’s LIFE program is Medicare/Medicaid eligible, which often offers financial relief to these families. It also serves an alternative solution to nursing home placement for those in need of medical assistance.

Participants have access to a team of dedicated physicians, specialists, nurses, social workers, aides, and dieticians, who are available 24/7, 365 days a year. Our goal is to help you live independently and remain in your home for as long as possible.
