LIFE is coordinated care.

What is LIFE?

LIFE – Living Independently for Elders – is Catholic Health’s PACE, or Program of All-inclusive Care for the Elderly.

This is an adult care program for participants that still live at home, but may be in need of more supervised care. Most seniors are hesitant to give up their independence, but may also need assistance to take care of themselves, participate in day-to-day activities, and remain feeling safe and secure.

LIFE offers both recreational and social activities for its participants. This allows adults to maintain a healthy and engaging lifestyle, while receiving personal care and supervision during the day.

Another advantage is that the Catholic Health team coordinates all medical care for participants of the LIFE program. Primary care services are provided by on-site physicians who are available at both of our locations. For comfort and convenience, medical services may also be provided in the participant’s home or at an off-site location. Transportation to and from medical appointments is included.

What LIFE Offers.

Our Services

As a LIFE member, you will have access to the services provided at both of our locations: The LIFE Center in Lackawanna, located on the campus of the OLV Senior Neighborhood; and the newly remodeled LIFE Center on the Villa Maria Campus in Cheektowaga.

How often you visit the center and the duration of your visit depends on your personal circumstances and medical needs. Visits typically range from one to five days per week.

Both LIFE Center locations are open Monday through Friday, from 8:30 a.m. to 4:30 p.m.
